Working Together to Support the Health of Greater Nashua

We are the greater Nashua Integrated Delivery Network (IDN)—a collaboration of community-based physical and mental health providers, substance use treatment providers and social service organizations who share a common goal – improving the health of those in our community who struggle with behavioral health issues. Our focus is to create a supportive network to coordinate care for those most at-risk, especially those individuals who rely on Medicaid for healthcare services.

Why are our resources important to the community?

By improving the community’s ability to coordinate resources and collaborate on programs that identify individuals in need, and providing access to appropriate services, the program is intended to reduce unnecessary use of hospital and emergency services due to untreated behavioral health needs and long wait times for treatment services.
